I cannot seem to get the smart taskbar to work with the above configuration -- any ideas?

Additional info...
On startup, or when trying to start the smart taskbar....I get this message:
The procedure entry point SetShellHook could not be located in the dynamic link library RTSUltraMonHook.dll

Sounds like you are using an old version of UltraMonTaskbar.exe with the update 1 version of RTSUltraMonHook.dll.
Reinstalling update 1 should fix this.

Actually I was using a fresh install of UltraMon B5 (just took out XP and went back to 2000 because XP does not like my new G450). I uninstalled UltraMon and then reinstalled and it worked this time.
