This could just be a configuration problem, but when I use the Smart Taskbar in standard mode, the taskbar icons on my primary monitor disappear. Please advise.

Do all task buttons get moved to the UltraMon taskbar? In this case, you would need to adjust the 'Show Tasks from' setting (right-click on the UltraMon taskbar).
The monitor which you want handled by the Windows taskbar needs to be unchecked.

I have a quad monitor setup. My primary monitor is monitor 2. No other taskbars have the monitor 2 icons. I verified that each monitor is only showing its icons. It was working great for as long as I've had it, and only recently has had this problem. I may just uninstall and reinstall it and see if that fixes it.
