The smart task bar occasionally shows programs that are no longer running. When mouseover the icon, it will show the name of the app, nothing when clicked (left and right).
One of the program that exhibits this behavior is Macromedia Dreamweaver MX. Also, most DirectX games leave a blank button on the smart task bar too.
Thank you!

Did you already adjust the taskbar priority as we discussed in this thread?

I have changed the priority of the taskbar according to the procedure you provided. The smart taskbar is very responsive, but still have the problem I described in the original post.
Thank you!

Is there some way to reproduce this problem, or does it occur randomly? I haven't noticed this problem so far (running WinXP).

The problem occurs whenever I finish playing a DirectX game, e.g. NHL2003, Need for Speed 6. It will leave an empty button on the smart taskbar when I quit the game. If I run the game again and then quit it, it will leave another button on it.
And I'm using a program called Auction Sentry (http://www.auctionsentry.com), which gives me the most problem. I'd have 10 icons of that program on my taskbar. I am not sure if there's a problem with this program but it sure is annoying.
Other than that, the program is great. Thank you for all your hard work!

I have this same problem. For certain programs run on the secondary monitor, their taskbar icon remains after the program is shut down. The only way I have found to make their taskbar icon go away is to disable the taskbar for that monitor, then re-enable it.

Yes, this problem is known to me. Seems to most frequently happen with Word 2002, but also Outlook 2002. Running on windows XP. Disabling and re-enabling the secondary monitor removes those icons.
